Allegations of Dr. Mary Elizabeth O’Connor’s professional misconduct have been referred to the Discipline Tribunal of the College. It is alleged that commencing in approximately October 2021, Dr. O’Connor engaged in disgraceful, dishonourable or unprofessional conduct and/or failed to respond appropriately or within a reasonable period of time to a written inquiry from the College, including by failing to co-operate with an investigation under s. 75(1)(a) of the Health Professions Procedural Code (“Code”) and failing to provide information, records and documents requested by College Investigative and Compliance Monitoring staff.
It is also alleged that between approximately November and December 2021, Dr. O’Connor breached a term, condition or limitation on her certificate of registration and/or engaged in disgraceful, dishonourable or unprofessional conduct, including by providing a vaccine exemption contrary to the terms of an Interim Order imposed by the Inquiries, Complaints and Reports Committee of the College under s. 25.4 of the Code, and by failing to comply with ancillary terms of the Interim Order, including failing to provide a signed OHIP consent form; practice details; patient logs; and other information requested by the College.
