             As  from  March  6,  2012,  the  following  is imposed as a term, condition and
            limitation on the certificate of registration  held by Dr. Sadan Kumar Goel, in
            accordance with an undertaking and consent given  by Dr. Goel to the College of
            Physicians and Surgeons of Ontario:

                  Dr. Goel may only engage in the following areas of practice:
            (i)   Surgical assist, only in hospital and not to be MRP for hospital patient;
            (ii)  Removal of lumps and bumps which will include  such  things  as  lypomas,
                  cysts,  moles,  skin  lesions  and  skin  tags.  Lymph  node  biopsy  for
                  diagnostic  purposes.  Skin  lesions  (Small Basal cell and Squamous cell
                  CA), simple excision or with rotation or  advancement  flap  to cover the
                  defect,  small  split thickness skin graft which can be done under  local
            (iii) Pilonidal sinus excision;
            (iv)  Carpal tunnel release procedure and release of trigger finger under local
            (v)   Suturing of skin  lacerations  and  excision  and exploration for foreign
            (vi)  Flexible sigmoidoscopy and rigid sigmoidoscopy.
            (vii) Upper endoscopy;
            (ix)  Vasectomy for family planning;
            (x)   Minor  ano-rectal  surgery suitable to be done in  office  setting  under
                  local anaesthesia, including  I&D of peri-anal and ischio-rectal abscess,
                  incision and removal of thrombosed haemorrhoids (perianal hematomas);
            (xi)  Haemorrhoid banding in office setting which does not need any sedation or
                  anaesthetic  and  sphincterotomy  for  acute  anal  fissure  under  local
            (xii) Excision of ganglion under local anaesthesia only;
            (xiii)Procedure involving toe nail surgery under local anaesthesia;
            (xiv) Intra articular injection of cortisone;
            (xv)  Removal of ano-rectal polyps, skin tags and condyolomata; and
            (xvi) Injection of varicose veins
