Undertaking by Dr. Thomas James Cairns, to the College of Physicians and Surgeons of Ontario (the “College”). (NOTE: This is a summary of the undertaking. For the complete terms, contact the College’s Registration Department):

I, Dr. Thomas James Cairns:

(2) Hereby undertake never to apply or re-apply for registration as a physician or for a licence to practice medicine in Ontario or in any other jurisdiction.
(4) Acknowledge that in exchange for this undertaking, the College agrees not to proceed with an investigation by the College pursuant to s. 75 of the Health Professions Procedural Code.
(5) Agree that in the event of a breach of any term of this Undertaking, the College shall in its sole discretion, have the right to proceed with a disciplinary proceeding on the basis of a breach of this undertaking and shall have the right to proceed with the s. 75 investigation in which Investigators were appointed on or about May 8, 2009, with respect to the events and subject matter which gave rise to the appointment of the said Investigators.
(7) Acknowledge and agree that if a referral of specified allegations to the Discipline Committee is made in relation to the terms of this Undertaking, the College will be entitled to introduce this Undertaking as evidence at a hearing before the Discipline Committee.
(9) Consent and understand that this undertaking shall be information on the College’s Register that is available to the public.
